SUBSTANCE ABUSE QUESTIONNAIRE - SHORT FORM
The SAQ-Short Form is used in many adult probation departments throughout the United States and some Canadian Provinces.
The SAQ-Short Form has been normed and standardized on thousands of defendants and substance (alcohol and other drugs) abuse clients.
In addition to alcohol and drugs, the SAQ-Short Form assesses other important areas of inquiry like truthfulness and risk (danger to self and others). This test is brief and to the point, yet provides important attitudinal and behavioral information.
The SAQ-SF has 64 items, takes 20 minutes to complete and has 4 measures (scales):
1. Truthfulness Scale
2. Alcohol Scale
3. Drugs Scale
4. Risk Scale
The Short Form is used in high volume testing settings with the reading impaired and as an alternative to the SAQ-Adult Probation III.
